Success Preparatory Academy is now accepting common applications for 2011-2012 in grades Kindergarten - 5th grade.

Applications are available for pick-up at the school (2011 Bienville Street).  They can also be downloaded from www.nolaparentsguide.org, or by clicking here.

What Do I Need To Enroll My Child In School?

REQUIRED

Please make sure you have these document ready when it's time to enroll your child in Success Preparatory Academy.

  • Birth certificate
  • Social Security Card
  • Immunization (shot) records
  • Individual Education Plan (IEP) for special needs students
  • Proof of local address (such as a electricity bill, telephone bill, copy of your lease)
  • Name, address and contact information for your child's last school

HELPFUL

These documents are not required but can help us work with your child.

  • Most recent report card
  • Last test results (IOWA, LEAP, other state tests)
  • Any other education records (especially if your child was out of school)

What if I don't have these?

Don't worry.  Complete your registration packet, and call us at 504-909-6275 for assistance.

Replacing Lost Documents

Social Security Cards:

call 1-800-772-1213 or visit www.socialsecurity.gov/ssnumber/.  You can get up to three replacement cards each year for free.

Birth Certificates:

If your child was born in Louisiana, call (504) 219-4500 or visit www.dhh.louisana.gov/offices/  If your child was born in another state, contact that state's Office of Vital Records.  For a fee, you can order copies of birth certificates online at www.vitalcheck.com

Immunization/Vaccination Records:

Call your local parish health unit.  In New Orleans contact the City of New Orleans Health Unit at 504-658-2510 or the regional office at 504-599-0100.

Driver's license or state ID:

If you need to replace your state identification or driver's license call the Department of Motor Vehicles at 1-877-368-5463 or visit http://omv.dps.state.la.us/
